What this report shows, the choices you make when running it, and how to read each section.
What this report is for
The Waiver Custom Fields report lists guests who signed a waiver within a date range, along with their address and the answers they gave to any custom questions on that waiver. It's useful when you need to pull together custom waiver answers — like emergency contacts, medical notes, or other information you've added to your waiver form — alongside basic contact details.
How the report works
You choose a date range based on when the waiver was signed, and can optionally search for a specific guest by first and/or last name. You also choose whether you want every waiver a guest signed in that range, or just their first or most recent one.
The choices you make when running it
These are the settings available. Everything else is calculated automatically.
1. The time period
| Setting | What it does |
|---|---|
| Start Signed Date | The beginning of the date range you want to report on, based on when the waiver was signed. |
| End Signed Date | The end of the date range you want to report on, based on when the waiver was signed. |
2. Who to include
| Setting | What it does |
|---|---|
| Guest First Name | Optionally search for a specific guest by first name. Leave blank to include every guest. |
| Guest Last Name | Optionally search for a specific guest by last name. Leave blank to include every guest. |
| Waivers to Display | Choose All Waivers Signed by Guest to include every waiver each guest signed in your date range, Last Waiver Signed by Guest to include only their most recent one, or First Waiver Signed by Guest to include only their earliest one. |
Columns on the report
| Column | Description |
|---|---|
| Waiver Group Id / Waiver Id | Internal identifiers tying the row back to the specific signed waiver record. |
| Guest Id | The internal identifier for the guest. |
| Guest Name | The guest who signed the waiver. |
| Street / City / State / Country / Zip Code | The guest's address on file. |
| Date Of Signature | The date the waiver was signed. |
How to read the layout
Each row is one signed waiver record for one guest. Beyond the columns above, a column is added for every custom question that was answered by at least one guest in your results, with that guest's answer shown in the matching column — so the exact set of columns you see will depend on which custom questions your waivers include and what guests actually filled in during your date range. If no signed waivers match your filters, the report will simply say so instead of showing an empty table.
